💼 About This Role

You'll conduct initial patient assessments for a leading home health agency serving 4,000+ patients across California. You'll ensure seamless Start of Care admissions while collaborating with an interdisciplinary team to deliver compassionate in-home care.

🎯 What You'll Do

• Conduct Start of Care (SOC) admissions and OASIS assessments
• Administer IV medications and monitor patient conditions
• Collaborate with physicians and clinical team for care plans
• Document observations and interventions accurately

📋 Requirements

• Active RN license in California
• Current BLS/CPR certification
• Valid CA driver's license
• Experience with OASIS assessments
