Requirements

• Valid Registered Nurse (RN) license in New York
• Home health or acute care experience preferred
• Ability to provide compassionate, patient-centered care
• Reliable transportation to client homes
• Flexibility to work day or overnight shifts as needed

Responsibilities

• Provide skilled nursing care to clients with complex medical needs
• Maintain patient safety and comfort in home setting
• Document patient care using electronic charting (Statewise)
• Collaborate with clinical team and family members
• Adhere to care plans and physician orders
