🎯 What You'll Do

• Conduct client assessments and care planning
• Oversee and educate RN case managers
• Make regular client visits to evaluate progress
• Supervise field staff and complete performance evaluations

📋 Requirements

• Active Registered Nurse license in state of practice
• Minimum 2 years recent related clinical experience
• At least 1 year in hospice, palliative, or home care
• Prior supervisory experience commensurate with program
