Description

You will make home visits to provide rehabilitative care and skilled nursing to adult and geriatric clients, helping them remain at home. You will develop individualized care plans, perform diagnostic tests, and educate clients and families.

Requirements

• Current New Hampshire Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA-accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of joints, strength, balance, etc.)
• Implement integumentary protective interventions for wound prevention and repair
• Develop and adjust individualized care plans with manual therapeutic exercises and gait training
• Educate clients and family members on rehabilitative care and independent living activities
• Document observations and interventions using an electronic touch pad tablet
