Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, conducting evaluations, developing individualized intervention plans, and educating patients and families to promote independence and safety.

Requirements

• Current Pennsylvania Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform home visits to evaluate patients' functional status, including muscle function, endurance, and ADL performance
• Develop individualized intervention plans to achieve favorable outcomes
• Identify equipment needs and environmental adaptations for enhanced independence
• Educate patients and families on rehabilitative care and activities
• Document patient care management accurately using a touch pad tablet
