Description

You will provide physical therapy treatments to home health patients under a licensed PT, helping them regain functional mobility and independence. You'll administer exercises, modalities, and educate patients and families, while documenting progress and collaborating with the care team.

Requirements

• Graduate of CAPTE-approved PTA program
• Minimum 3 years full-time PTA experience or 2,000 patient care hours
• Current Ohio PTA license or application in process
• Valid Ohio driver's license and reliable transportation with insurance
• Current CPR certification

Responsibilities

• Provide physical therapy services per Home Health Plan of Care under licensed PT supervision
• Administer therapeutic exercises, massage, traction, and modalities
• Assist patients in improving functional mobility and independence
• Observe, document patient progress, and report changes to supervising PT
• Prepare clinical documentation and progress notes in a timely manner
