You'll provide intensive case management to clients with complex needs who are sleeping rough or residing in Dandenong, helping them secure and sustain housing. You'll collaborate with a multi-disciplinary team to develop goal-oriented case plans and connect clients to community resources. • Engage rough sleepers and provide intensive case management

📋 Requirements

• Relevant tertiary qualification in human services (desired)
• Experience delivering services to clients with complex needs
• Understanding of housing and homelessness policy and systems
• Valid Victorian Driver's License
