You'll provide hands-on therapeutic interventions and patient evaluations in a home health setting. You'll build core clinical skills with strong mentorship from a supervising PT Case Manager. This entry-level role focuses on follow-up visits rather than case management.

🎯 What You'll Do

• Provide hands-on therapeutic interventions like gait training and manual therapy.
• Conduct detailed patient evaluations in the home environment.
• Document patient care accurately within 24-48 hours.
• Collaborate with interdisciplinary team and supervising PT Case Manager.

📋 Requirements

• Current California Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License with clean record.
• Graduate of an accredited Physical Therapy program.
