You'll coordinate the PACE eligibility and enrollment process, converting inbound inquiries to home visits. Your work will directly support our mission of transforming senior care through an all-inclusive healthcare option.

🎯 What You'll Do

• Answer calls and conduct follow-up calls to referrals
• Assign possible enrollments to Benefits Coordinator for financial verification
• Schedule initial home visits and assessments
• Assist MOEE Director in meeting facilitation using Salesforce

📋 Requirements

• Associate's degree in a relevant field
• 2 years of relevant experience
• Strong customer service orientation
• Experience with Salesforce

• Bachelor's degree
• Bilingual
• Experience with MS Office Suite or Athena
