💼 About This Role

You'll provide hands-on therapeutic interventions in patients' homes under the supervision of a PT Case Manager. You'll build core clinical skills while contributing to our top-ranked home health agency. This role offers strong mentorship and professional development in a supportive environment.

🎯 What You'll Do

• Provide hands-on therapeutic interventions like gait training and manual therapy.
• Conduct detailed patient evaluations and assessments in the home environment.
• Implement individualized plans of care under supervision of a PT Case Manager.
• Document patient care accurately within 24-48 hours.

📋 Requirements

• Current unrestricted California Physical Therapist (PT) License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.

• One year of experience as a Physical Therapist.
• Prior home health experience.
• Experience treating high acuity patients.
