Description

You will provide basic nursing care, assess residents, implement care plans, administer medications and treatments, and monitor changes in condition in a rehab/long-term care setting. You'll collaborate with the interdisciplinary team and communicate with residents and families.

Requirements

• Graduate of accredited LPN school
• Current LPN license in state
• IV certification or willingness to obtain within 6 months
• Current CPR/AED certification or willing to obtain
• Long-term care/rehab/assisted living experience preferred

Responsibilities

• Assist in resident assessment on admission and ongoing
• Participate in care planning and update care plans
• Administer medications and treatments safely and timely
• Notify physician of changes using SBARR tool
• Monitor care plan delivery and identify problem areas
