💼 About This Role

You'll provide skilled, compassionate home health nursing care to seniors under CMS guidelines. You'll collaborate with an interdisciplinary team to coordinate care plans that empower participants to live healthier, more independent lives. This role skips OASIS assessments and offers hourly pay including travel time.

🎯 What You'll Do

• Conduct home health nursing assessments and determine equipment needs.
• Perform in-home urgent care assessments and point-of-care testing.
• Maintain on-call readiness and respond promptly to visit requests.
• Deliver and document nursing interventions per care plans.

📋 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN License.
• Minimum of one (1) year of nursing experience with frail or elderly population.
• Reliable means of transportation.

✨ Nice to Have

• Home health agency experience preferred.
• CPR and First Aid certification upon hire preferred.
