Description

You will play a critical role in our whole-person, interdisciplinary care model, outreaching and engaging individuals with serious mental illness, substance use disorder, homelessness, or high medical needs. This role is primarily remote with 2-3 on-site visits per month in the community.

Requirements

• Bilingual in Spanish and English
• Passion for serving underserved populations
• Creative and empathetic thinking
• Willingness to work remotely with periodic in-person visits
• Experience in case management or healthcare preferred

Responsibilities

• Outreach and engage individuals with serious mental illness, substance use disorder, homelessness, or high medical needs
• Conduct on-site community visits 2-3 times per month
• Build trust and relationships with underserved populations
• Collaborate with interdisciplinary care team
• Support whole-person care including clinical, behavioral, and social needs
