💼 About This Role

You'll perform home health evaluations and develop patient care plans to help individuals regain independence. Working with an interdisciplinary team, you'll restore function and prevent disability following disease or injury, with autonomy to create your own schedule.

🎯 What You'll Do

• Perform in-home patient evaluations and develop care plans
• Help patients with mobility and regain independence
• Communicate and coordinate with patients' physicians
• Evaluate and report patients' progress to physicians

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
