You'll serve as the primary liaison between PACE participants and their interdisciplinary care team, ensuring smooth onboarding and ongoing support. You'll drive service recovery initiatives and satisfaction surveys to enhance the participant experience. This role directly impacts vulnerable seniors' ability to stay in their homes and communities.
π― What You'll Do
- Maintain open communication with interdisciplinary team and external providers
- Assist with participant experience and service recovery special projects
- Facilitate communication among service providers, families, and caregivers
- Provide customer service calls to ensure satisfaction with appointments
π Requirements
- High school diploma or equivalency
- Excellent organizational and communication skills
- Ability to present information to various groups
- Must obtain CPR and First Aid certification within 180 days

- Bachelorβs degree in communications or relevant field
- CPR and First Aid certification upon hire
