You'll serve as the attending physician for patients in skilled nursing facilities, collaborating with NPs/PAs to manage daily care. 🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and clinical supervision.
• Diagnose and treat acute and chronic conditions in a patient-centered setting.
• Participate in Chronic Care Management, Transitional Care, and Remote Patient Monitoring initiatives.

📋 Requirements

• Active U.S. medical license (MD/DO).
• Board-certified or board-eligible in Internal Medicine, Family Medicine, Emergency Medicine, or related specialty.
• Ability to perform essential functions with or without reasonable accommodation.

• Geriatric Medicine experience or certification.
• Experience in skilled nursing facility or post-acute care.
