Description

As a Jefferson Health at Home by BAYADA Wound, Ostomy and Continence Nurse, you will provide 1:1 skilled nursing care to patients in their homes in Montgomery County. You'll conduct wound care consultations, perform assessments, administer medications and treatments, and educate patients and families on managing their conditions—all while helping patients transition from hospital to home with comfort and dignity.

Requirements

• Active RN license in Pennsylvania
• Wound, Ostomy and Continence Nurse (WOCN) certification
• At least one year of nursing experience
• Ability to travel to cases as assigned
• Strong assessment, communication, and active listening skills

Responsibilities

• Deliver 1:1 skilled nursing care including medication administration, wound care, and treatments
• Conduct wound care consultations and provide oversight
• Perform client assessments and monitor patient condition
• Provide education to patients and families
• See patients through care needs from hospital discharge to safe home life
