Description

You will directly engage with individuals living with serious mental illness, substance use disorder, or high medical needs, building trust and coordinating whole-person care. This role is primarily remote with 2-3 on-site community visits per month, empowering underserved populations through a tech-enabled care model.

Requirements

• Bilingual in Spanish and English
• Passion for serving underserved populations
• Experience in case management or care coordination
• Ability to work remotely and travel locally for community visits
• Strong interpersonal and communication skills

Responsibilities

• Outreach and engage individuals with serious mental illness, substance use disorder, or high medical needs
• Coordinate whole-person clinical, behavioral, and social care
• Participate in on-site community visits 2-3 times per month
• Leverage AI and automation tools for care coordination
• Collaborate with interdisciplinary team of nurses, social workers, and community health workers
