💼 About This Role

You'll coordinate care for individuals with complex needs, acting as the central point of contact to ensure seamless communication across medical, behavioral, and social services. You'll help clients achieve more quality days at home through person-centered care planning and advocacy.

🎯 What You'll Do

• Assess member needs in physical health, mental health, and social supports
• Develop and oversee client care plans and SMART goals
• Conduct outreach and engagement activities to facilitate program linkage
• Advocate on behalf of clients with health care professionals

📋 Requirements

• Active ACSW, LCSW, LMFT, or LPCC license in California
• 1+ year of experience as a care manager or community health worker
• Valid driver's license, clean driving record, and reliable transportation
• Working knowledge of government and community resources related to social determinants of health

✨ Nice to Have

• Bilingual English/Spanish
• Experience with Motivational Interviewing and Trauma-Informed Care
• Knowledge of Google Workspace and MS Office
