Description

You will make home visits to provide rehabilitative care and skilled nursing to adult and geriatric patients, performing evaluations and developing treatment plans for speech, language, communication, or swallowing difficulties. You'll work as part of a multi-disciplinary team to help clients remain at home while receiving necessary medical care.

Requirements

• Current Speech Language Pathologist license in Colorado
• Certificate of clinical competence in speech from ASHA or equivalent educational/work experience
• At least one year of experience as an SLP
• Prior home health experience is a plus

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician orders
• Educate patients and family members on rehabilitative care for health, safety, and independent living
• Document observations and interventions using a touch pad tablet
