Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, performing diagnostic tests, developing care plans, and educating clients and families as part of a multidisciplinary team.

Requirements

• Current MD Physical Therapist license
• One year of recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP approved program
• Ability to work independently and manage time effectively

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, strength, balance, etc.)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans
• Educate clients and families on rehabilitative care
