💼 About This Role

You'll join a compassionate team dedicated to transforming care for individuals with complex needs. As a Clinical Lead Care Manager, you'll serve as the primary point of contact, coordinating with doctors, specialists, and social services to ensure holistic care. This role offers the chance to make a profound impact through comprehensive care management and advocacy for vulnerable populations.

🎯 What You'll Do

• Assess member needs in physical health, mental health, and social supports.
• Develop and oversee client care plans and goal settings.
• Coordinate care with providers and community services.
• Maintain documentation and patient health records in EMR systems.

📋 Requirements

• Active unrestricted LCSW, LMFT, or LPCC license in California.
• 1+ year experience as a care manager or care navigator for vulnerable populations.
• Working knowledge of government and community resources for social determinants of health.
• Available Monday-Friday 8:30am-5:00pm Pacific Time.

✨ Nice to Have

• Bilingual (English/Spanish).
• Experience with motivational interviewing or trauma-informed care.
