Description

You will provide home health speech therapy to adult and geriatric patients in Vance, Granville, Franklin, Nash, and Warren counties. As a key member of a multi-disciplinary team, you'll perform evaluations, develop treatment plans, educate patients and families, and document care using a tablet.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence in Speech from ASHA or equivalent education/experience
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician orders
• Educate patients and family members on rehabilitative care and activities
• Document interventions and evaluations using an electronic medical record tablet
