You'll provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss care to patients through chat, phone, and video calls. You'll collaborate with physicians and Patient Care Advocates to ensure patient success in a virtual care setting. 🎯 What You'll Do

• Provide comprehensive diabetes, hypertension, hyperlipidemia and weight loss care via chat, phone, and video
• Create thorough consults aligned with internal protocols for physicians
• Collaborate with Patient Care Advocates to ensure patient success

📋 Requirements

• Registered Dietitian Nutritionist plus CDCES or BC-ADM or eligible
• Experience in medication adjustment
• Experience using an EMR
• Dedicated quiet workspace
