💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes. Your work enables clients to receive medical care at home, improving their quality of life. You'll be part of a supportive multi-disciplinary team with career advancement opportunities.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility, strength, and balance.
• Implement integumentary protective interventions for wound prevention and repair.
• Develop and adjust individualized care plans including therapeutic exercises.
• Educate clients and families on rehabilitative care and activities.

📋 Requirements

• Current VA Physical Therapist license.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved program.

• Prior experience with electronic medical records (EMR).
• Home health experience preferred.
