Description

You will lead our Assistive Care State Programs office, providing clinical oversight, conducting client assessments, developing care plans, and supervising Home Health Aides to help adults and seniors remain safely at home in Hudson County.

Requirements

• Active RN license
• 2+ years of RN experience
• Home care or supervisory experience preferred

Responsibilities

• Lead, inspire and motivate team to provide exceptional care
• Provide clinical oversight for assistive care state program clients
• Conduct client assessments and develop individualized care plans
• Supervise, mentor, and support Home Health Aides
• Facilitate new hire orientation
