Description

You will perform home health visits for adult and geriatric patients in Independent and Assisted Living communities. Your role includes evaluating functional status, developing intervention plans, and educating patients to promote independent living.

Requirements

• Current DE Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations of patients' functional status including muscle function, endurance, and ADL/IADL performance
• Assess cognitive, psychological, and motor aspects and develop individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and family members on rehabilitative care and document using touch pad tablet
