🎯 What You'll Do

• Conduct initial and follow-up in-home assessments on flagged patients
• Train caregivers on using Circadia into clinical workflows
• Interpret trend-based insights to prioritize high-risk patients
• Complete documentation within 24 hours using AI-powered tools

📋 Requirements

• Certified Nurse Practitioner in CA with ANCC certification
• 2+ years NP experience in home health or post-acute settings
• Valid CA driver's license and reliable transportation
• Comfortable with face-to-face in-home visits for complex care

✨ Nice to Have

• Google Suite, multiple EMRs (HCHB a plus), Slack, AI tools
• Spanish language skills
