🎯 What You'll Do

• Deliver nursing services as per individualized care plans
• Collaborate in interdisciplinary planning and case conferences
• Record and communicate patient observations accurately
• Educate patients and caregivers about palliative care and symptom management

📋 Requirements

• Active California LVN license
• 2+ years of clinical nursing in field-based setting
• Valid driver's license, insurance, and reliable vehicle
• Proficiency in EMR and Google Suite

• Hospice or palliative care experience
• Bilingual skills
