Description

You will act as a bridge between patients, community resources, and healthcare services to remove barriers and increase wellness. You'll work with high-risk patients to identify health goals, coordinate care, and connect them to resources like housing, transportation, and food. This role involves proactive in-person and virtual outreach, driving throughout Seattle to meet patients in their homes and communities.

Requirements

• At least two years of patient-facing work experience in social services, community-based case management, or healthcare outreach
• Strong verbal and written communication skills
• Proficient in Google Workspace and electronic medical record systems
• Access to reliable transportation and ability to travel throughout Seattle

Responsibilities

• Establish and maintain relationships with patients, community organizations, and team members to coordinate services
• Identify and initiate referrals for social service programs including financial, psychosocial, and community resources
• Manage resource-focused care plans as part of patient's continuum of care
• Connect patients to community resources related to housing, transportation, food, and daily living activities
• Promote goal setting and achievement to improve patients' quality of life and self-efficacy
