Description

You will provide basic nursing care, including medication administration and treatments, while monitoring residents' conditions and communicating with the interdisciplinary team. Your role is crucial in ensuring safe, timely care and contributing to care planning for geriatric residents.

Requirements

• Current LPN license in state of work
• CPR and AED certification (or obtain within 6 months)
• IV certification (or obtain within 6 months)
• Ability to assess nursing care needs of geriatric residents
• Ability to stand/walk for long periods, lift heavy objects, and transfer patients

Responsibilities

• Assist in resident assessments on admission and ongoing
• Administer medications and treatments safely and timely
• Participate in care planning and update care plans as needed
• Communicate changes in condition to interdisciplinary team and physicians using SBARR
• Monitor care delivery and address problem areas using QAPI process
