💼 About This Role

You'll provide rehabilitative care and hospice visits for adult and geriatric clients in Gloucester, Cumberland, and Salem Counties. As a home care Physical Therapist, you'll be an integral member of a multi-disciplinary health care team.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility and strength.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care.

📋 Requirements

• Current New Jersey Physical Therapist license.
• Minimum of one year recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association.
