💼 About This Role

You'll conduct in-home care assessments and coordinate personal care services for seniors enrolled in a PACE program. You'll collaborate with an interdisciplinary team to ensure participants thrive in their homes. This role offers a hybrid schedule and flexible workday.

🎯 What You'll Do

• Coordinate incoming calls regarding participants, physicians, and agency services
• Communicate schedule changes and updates with participants and care team
• Track and monitor home care delivery and scheduling hours
• Assist with staffing and scheduling activities
• Participate in end-of-life care coordination and support

📋 Requirements

• Healthcare/medical licensure or equivalency
• At least 3 years of professional experience
• Minimum of 3 years of case management or nursing experience with frail/elderly population

• Bachelor's degree
• Nursing knowledge and training for frail elderly participants
• Care planning and management experience in home care
