Requirements

• Active physical therapy license in good standing
• BLS CPR certification
• Reliable transportation for patient home visits
• Basic comfort with technology (email, texting, EMR)

Responsibilities

• Deliver in-home physical therapy to adult and geriatric patients
• Evaluate patient mobility, strength, and function for individualized goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate home health care documentation per Medicare standards
