💼 About This Role

You'll provide skilled home health physical therapy services to patients in the Antioch area, helping them regain independence and mobility. Your work directly contributes to our industry-leading low readmission rates and high employee satisfaction. This part-time role offers flexible scheduling, targeted territories to minimize driving, and a supportive team culture.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans.
• Help patients with mobility and independence goals.
• Coordinate with patients' physicians and report progress.
• Supervise Physical Therapy Assistants as needed.

📋 Requirements

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.

• Home health experience highly preferred.
• Case management fundamentals highly desired.
• Experience with interdisciplinary teams desired.
