💼 About This Role

You'll care manage and transition complex, chronically ill clients from alternative care settings to home while educating referral sources about BAYADA Home Health Care. You'll build referrals and long-term relationships, ensuring high-quality and safe transitions. This hybrid role requires regular travel through Southern NJ, DE, and Southeast PA.

🎯 What You'll Do

• Develop yearly marketing plan and strategic quarterly goals
• Educate and engage prospective clients and families about home care
• Promote effective care coordination with facilities and communicate clinical status
• Provide post-transition home follow up telephonically for 2 months

📋 Requirements

• Licensed Registered Nurse in respective state
• 5 years experience in nursing, discharge planning or home care
• 4-year college degree
• Basic PC skills (Word, Excel, SharePoint, Outlook)

✨ Nice to Have

• Experience in pediatrics or rehabilitation
• Experience in discharge planning
• Verbal and written communication skills
