Description

You will provide skilled professional nursing care to home health patients as prescribed by the physician, in compliance with Medicare conditions and Ohio licensure laws. This role involves traveling locally to patient homes and working Monday-Friday with some on-call rotation.

Requirements

• Graduate of an approved nursing program
• Valid Ohio LPN license
• CPR certification
• Minimum 2 years direct patient care in hospice or home care
• Valid driver's license, auto insurance, and clean driving record

Responsibilities

• Provide skilled nursing care to home health patients as prescribed by physician
• Manage primary caseload for skilled home health care patients
• Adhere to Home Health Medicare Conditions of Participation and Ohio licensure laws
• Comply with agency policies and procedures
