💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric patients in senior living communities. You'll perform home visits and develop treatment plans to help clients remain at home. Prior home care experience is a plus but not required.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and family members on rehabilitative care activities.

📋 Requirements

• Current license as a Speech Language Pathologist in VA.
• Certificate of clinical competence from the American Speech and Hearing Association.
• Solid computer skills; electronic medical records experience preferred.

• Prior home care experience as an SLP.
• Experience with electronic medical records (EMR).
