Description

You will provide personal care to hospice patients, observing and reporting changes in their condition, and supporting their families. You'll work as part of a multidisciplinary team to ensure comfort and quality of life for terminally ill patients.

Requirements

• Minimum age 18
• CNA/STNA certification on Ohio State Tested Registry
• CPR certification
• 1+ year direct patient care experience
• Valid driver's license and insured vehicle

Responsibilities

• Provide personal care with respect and dignity
• Observe, record, and report patient's care, symptoms, and status
• Assist with feeding as needed
• Communicate with patient/family and agency personnel
• Attend in-services, meetings, and IDT meetings
