Description

You will directly outreach and engage individuals with serious mental illness, substance use disorder, or high medical needs, helping them access whole-person care. This role involves building trust and relationships to connect underserved populations with clinical, behavioral, and social support.

Requirements

• Bilingual Spanish speaking
• Highly motivated and empathic
• Willingness to think creatively
• Experience or interest in serving underserved populations
• Comfort with remote and occasional on-site work

Responsibilities

• Outreach and engage individuals with SMI/SUD, homelessness, or high medical needs
• Build trust and relationships to connect patients with care
• Perform 2-3 on-site visits per month
• Collaborate with interdisciplinary care team
• Support whole-person care model
