Description

You will provide home health occupational therapy visits to adult and geriatric patients in Somerset County. As part of a multidisciplinary team, you'll evaluate functional status, develop individualized care plans, and educate patients and families to promote independent living.

Requirements

• Current New Jersey Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of functional status including muscle function, endurance, and ADL/IADL performance
• Develop and revise individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards and identify equipment needs or environmental adaptations
• Educate patients, families, or representatives in rehabilitative care to promote health, safety, and independent living
