💼 About This Role

You'll provide home health visits to adult and geriatric clients in Southern Chester County, performing diagnostic tests and developing individualized care plans as part of a multi-disciplinary team.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility and strength.
• Develop and implement individualized care plans with manual therapy.
• Educate clients and families on rehabilitative care and safety.

📋 Requirements

• Pennsylvania PT license
• 1 year recent experience as a Physical Therapist
• Graduation from an APTA-accredited program
• Solid computer skills for documentation
