You'll join a multi-disciplinary team providing home health visits to clients in Ocean County. You'll evaluate social and emotional factors, plan interventions, and connect clients with community resources. This role offers the chance to work with a supportive team using a state-of-the-art touch pad tablet.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Provide clinical social evaluations and plan appropriate interventions
• Identify appropriate community resource referrals
• Accurately document observations and interventions using a tablet

📋 Requirements

• LMSW or LISW license from NJ Board of Social Work Examiners
• Masters in Social Work with accreditation by Council on Social Work Education
• At least 2 years of community-based experience
