💼 About This Role

You'll provide home health visits to adult and geriatric clients in Lebanon County, performing diagnostic tests and implementing individualized care plans. You'll be an integral member of a multi-disciplinary team helping clients remain at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, and balance.
• Develop and implement individualized care plans including therapeutic exercises.
• Educate clients and family members on rehabilitative care and safety.

📋 Requirements

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by APTA or CAAHEP.
• Solid computer skills with electronic medical records (EMR) experience preferred.
