Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, as part of a multi-disciplinary team. Your work enables clients to receive necessary medical care while remaining at home.

Requirements

• Current VA Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP accredited program
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans including manual therapy and gait training
• Educate clients and families on rehabilitative care for health and safety
