Description

You'll provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes throughout York County. As a key member of a multi-disciplinary team, you'll perform evaluations, develop treatment plans, and educate patients and families to promote health, safety, and independent living.

Requirements

• Current Speech Language Pathologist license in Pennsylvania
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational requirements
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician orders
• Educate patients and family members on rehabilitative care and activities
• Document observations and interventions using electronic medical records
