💼 About This Role

You'll be Waymark's frontline presence in McHenry County, connecting patients from low-income communities with social support and resources. You'll conduct needs assessments, help with health-related social issues, and manage relationships with healthcare providers and community organizations. This role offers paid training and the opportunity to work with a multidisciplinary care team.

🎯 What You'll Do

• Meet patients in community, home, and healthcare facilities for needs assessments.
• Help patients with health-related social issues like homelessness and hunger.
• Manage relationships with healthcare providers and care delivery team.
• Document each patient encounter in technology systems accurately.

📋 Requirements

• Reside within a commutable distance of McHenry County, Illinois
• Available to work a set schedule including at least one weekend day per week
• Ability to travel up to 80% within surrounding counties
• Current driver's license and access to an insured vehicle

• Community Health Worker certification
• Long time resident of McHenry County with knowledge of community resources
