Description

You will provide after-hours hospice nursing care to patients in their homes, performing assessments, administering medications, and supporting families. Your role ensures continuity of care and coordinates patient needs with the interdisciplinary team.

Requirements

• Bachelor's or Associate's degree in nursing or related field
• Active nursing license in good standing in Ohio
• Active CPR certification
• 2-5 years of experience in healthcare management, hospice, or palliative care preferred
• Proficient in MS Office applications and ability to learn software systems

Responsibilities

• Complete initial and comprehensive assessments of patients and families to determine hospice needs
• Provide professional nursing care, including physical assessments, medication administration, and symptom management
• Develop and revise care plans based on nursing diagnosis, incorporating palliative nursing actions
• Support case managers and coordinate patient care after hours
• Document patient condition and care given in clinical notes, and communicate with physicians
