You'll lead BAYADA's Transitional Care Program for skilled nursing within the Private Duty Nursing division, ensuring high-level, medically complex care in the home. Your core impact will be developing and managing the program to drive strategic growth through market research, business development, and referral relationships.

🎯 What You'll Do

• Design and implement the Transitional Care Program.
• Train transitional care managers and family caregivers.
• Manage TCP metrics and performance outcomes.
• Build and maintain referral source relationships.

📋 Requirements

• Four-year college degree in nursing, marketing, or related field.
• Two years supervisory or management experience in healthcare.
• Formal sales training and ability to implement a sales plan.
• Registered Nurse license strongly preferred.
