Description

You will provide end-of-life care to hospice clients in their homes across York County and surrounding areas. You'll manage medications, wounds, treatments, and comfort care while monitoring client conditions and documenting care.

Requirements

• Current Registered Nurse (RN) license in Pennsylvania
• Minimum 1 year of recent, verifiable clinical (medical/surgical) experience
• Ability to travel to cases as assigned
• Prior home care and/or hospice experience preferred
• Solid computer skills; prior experience with electronic medical records (EMR) preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, procedures, and comfort care
• Monitor client conditions and report changes to Clinical or Client Services Manager
• Execute and document doctors' orders
• Perform client assessments as necessary
